Co-pays are due at time of service. Lewis is a Board-Certified Psychiatric Mental Health Nurse Practitioner. He received his Bachelor of Science in Nursing from East Tennessee State University College in 2009 and in 2013, Lewis received his Master's of Science in Nursing from the University of Cincinnati.

Milestone Psychiatry understands that each individual is unique in their presentation and that each treatment plan needs to be tailored specific to that individual. We focus on your personal needs, goals and experiences to design a treatment plan that will help you grow and flourish. We provide mental health medication management in a safe and welcoming environment for patients 5 years and older.
New Patients

We are happy to see that you have decided to take the first step to seeking help for you and/or your loved ones. Prior to your first appointment, you must complete the New Patient Intake documentation. If you are coming to the session in person, you may complete this at the office, but we ask that you arrive 10-15 minutes before your scheduled time to complete these forms to ensure you get the complete time allotted for the appointment.
