If you are unable to make a scheduled appointment for any reason including sickness and emergency, 24 hour notice is required via phone or email. An $50 fee will be charged if 24 hour notice is not received. The practice is located in the ActivSpace building, which is on the corner of 18th and Treat (3150 18th street 94110).
