For privately insured patients, upon request, we can provide the necessary information for you via a coded superbill to file a claim with your insurance company (there is a small charge to cover the physician's time).

Consents and ID (state-issued ID and insurance ID) must be sent via Klara at least two (2) business days prior to the visit. We will use video chat (similar to Skype or FaceTime) to review your medical history and look at your skin/ hair/ nail issue. We will send you questions prior to your visit and request clear images of the issue.
