Your Trusted Community Health Partner in San Francisco
San Francisco Community Health Center is a nonprofit organization dedicated to transforming lives through advancing health, wellness, and equality. Founded in 1996, we provide comprehensive medical, dental, and mental health services tailored to immigrant communities, people of color, LGBTQ+ individuals, unhoused populations, and others impacted by systemic oppression. Our client-centered approach prioritizes healing justice, addressing social determinants of health, and fostering belonging through culturally informed care designed by the communities we serve. We operate as a Federally Qualified Health Center, guided by core values of compassion, justice, leadership, respect, sex positivity, and wellness. Our programs include Trans: Thrive, Street Medicine, and the Taimon Booton Navigation Center, alongside policy advocacy and community-building initiatives.

We maintain locations in San Francisco's Tenderloin, Little Saigon, and Polk Street neighborhoods to ensure accessibility. Our team combines clinical expertise with lived experience to deliver affirming, whole-person care that meets basic needs while addressing structural inequities. We measure success through community impact, with 16,000 lives touched in 2025 alone. Our work extends beyond clinical services to include movement-building, policy advocacy, and partnerships that advance health justice. We remain committed to innovation, evidenced by our 2015 recognition as a Federally Qualified Health Center and ongoing expansion of trans-affirming care and long-acting treatment options.
