It is our goal at Trinity Family Medical Center to make your visits at our office as pleasant, comfortable and worry free as possible. In order to do this we have established certain office policies to enable us to have the best possible relationship with our patients. Scheduling and Cancellations: As a courtesy to our patients we make reminder appointment phone calls/e-mails 24-48 business hours before your appointment.

The purpose of the portal is to provide our patients with the tools they need to communicate and interact with our staff over the internet in hopes that the patient/provider relationship will be strengthened. The portal will allow patients to see scheduled appointments, request appointments, process demographic and insurance changes, request refills, ask the nurse a question and in the near future have access to our actual health information.
