Greta reviews all the collected information and formulates a written ACTION PLAN based on your stated goals and your active issues. 5. Schedule a 1/2 hour follow-up appointment with Greta to go over your personalized ACTION PLAN.
Comprehensive Medication Management

CMM is a critical review of a patient's health and medication profile to help identify duplications, omissions, interactions, side effects and other opportunities for optimizing and improving treatments. This comprehensive service includes an hour-long interview and a 30-minute follow up session to go over a report of findings and recommendations.

Mr Smith takes care of Mrs Smith who has heart failure and takes 8 different medications throughout the day. Lately it has gotten harder for Mr. Smith to organize the medications for his wife and he needs some help.

Allow the pharmacy staff to stagger other tasks so they can give you prompt attention when you arrive. Please plan to remain in the pharmacy for 15 minutes post-vaccine so that we may observe you for possible reactions.
