Primary Care
Our primary care services provide comprehensive health management for all ages, including preventive check-ups, chronic disease treatment, and acute illness care. We offer onsite lab services and coordinate with specialists for complex conditions. Patients benefit from consistent care that tracks their health history over time, with flexible scheduling options and telehealth availability.
Behavioral Health
Integrated behavioral health services include individual and family counseling, medication management, and group therapy for mental health conditions. Our licensed professionals treat anxiety, depression, addiction, and ADHD, with specialized programs for children and crisis intervention. Services are available at select locations with referrals to appropriate specialists when needed.
Pediatrics
Pediatric care covers newborn to age 18 with wellness visits, vaccinations, sports physicals, and acute illness treatment. We offer after-hours care guidance and MyChart access for parental record management.
OB/GYN Services
Comprehensive women's health services include annual exams, prenatal care, and management of gynecological conditions. We provide contraceptive counseling, STD testing, and menopause support at all locations, with full prenatal services including ultrasounds available at Shadeland and Anderson locations.
Dental Services
Dental care includes routine cleanings, fillings, and extractions at two locations. We serve Medicaid and self-pay patients with pediatric dental exams starting at age 1. Complex procedures like crowns and implants are available through partner organizations, with sliding fee discounts based on income for uninsured patients.

I have been going here for 9 years now and have never had a bad experience with my care team. Although the check-in ladies are most of the time rude and short.

The doctor's visit was not what I expected. They did diabetes test which I did not want. The doctor: Hubert Dolubizno, DO had me wait exceptional long. Longer than what he told me. Finally, he didn't perform physical (hernia) that I requested. That was whole point. He was "offputting". I got the impression that he just didn't want to or was not comfortable. That will be last time I visit that place and with that physician!

Rude staff, takes months to see the doctors here for follow up, inconsistent with how they run things. Don't go here, you can find better experiences elsewhere. With what medince in general costs there is no excuse for their behavior.

I went today for my first OB appointment. They got me in within 5 mins And I was there 15 mins early. Everyone I came in contact with had a pleasant attitude and was super nice! I waited 8 minutes tops in the back between visit and lab session. Overall very pleased with everything and everyone.
