Medications: Bring all of the medications you are taking with you. Bring the actual containers of the medications or a list of the medicatiosn including the dosage and the number of times a day the medications are taken. This includes over-the-counter medications. List of Concerns: Prior to the visit, write a list of the issues that you have and would like to discuss with your health care provider.
