We will work with your home health, hospice, physical therapy, occupational therapy, speech therapy, and other ancillary staff to assure we provide the best care. House calls and Weekend Visits (extra fees apply & offered within a 5 mile radius of our clinic). We strongly advise you to participate in our Medicare LiveActive Chronic Care Management (CCM) Program, which is COVERED by Medicare. Transition of Care management: We will contact you within 2 business days following a specialist visit or hospital discharge.
