To help expedite the registration process, we have uploaded our New Patient Registration resource forms. In an effort to respect the time of all our patients, our staff strives to stay on schedule. For patients who are delayed or arrive late for their appointment, every effort will be made to see you the same day.
Instructions Before Procedures/surgery

Before you head to our office for your procedure, take a look at our recommended pre-operation instructions below. Please notify our staff two days before your procedure if you are taking medication for diabetes, heart, or blood-thinners (e.g., Heparin, Coumadin, Warfarin, Plavix, etc.).
The Surgery Center where your procedure will be performed will contact you two days prior to your appointment to provide pre-op instructions and any additional information.Make sure to thoroughly shower the night before and the morning of your procedure to reduce the risk of infections.

If you have soreness, please take your regular pain medications. If you do not have prescribed pain medications, you may take 2 Extra Strength Tylenol (1 gram) every 8 hours over the next 24 hours. You may apply moist heat the following day as needed and warm showers in the morning and evening. Use the heating pad with caution and on low setting only;.

Patients must be located within a 30-mile radius of our Minivasive Pain & Orthopedics offices in order to qualify for transportation. Patients will not be given a pickup time when setting up an appointment. They will be given a call a day before (which they have to answer) to confirm and be given a pickup time.
