Home Health

Providing quality home health services to those who wish to stay in the place they call home while dealing with an injury, illness, or recuperating from a recent hospital stay. At Companion Healthcare, your health, safety and comfort are our greatest concerns. Our team works closely with your physician to tailor a plan of care specifically for you.
Special Programs

Vital signs are electronically sent to our office for daily review, allowing our clinicians to recognize subtle changes and to intervene early. The Telemonitor includes standard languages: English, Spanish, French, German, Polish, Russian, Italian and many more. Provides voice commands and light cues that will remind you or your loved ones when it is time to take medications.
Hospice

Hospice care is available to those facing a life-limiting illness. Throughout this journey our experienced and sensitive team will provide meaningful care and support to your loved one, as well as your family. Our hospice team will be there explaining the process, addressing physical, emotional, and spiritual concerns.
Services Included and Payer Sources

Companion Hospice offers an individualized plan of care that is developed by the hospice team, the physician, and the patient and family to address the patient's specific needs. The hospice team responsibilities include the management of the patient's pain and symptoms and assisting the patient and family with the emotional, psychosocial, and spiritual concerns when dealing with end-of-life care.
