Willow Creek Nursing Home is a 24-hour licensed long-term care facility. We serve persons who may have a chronic illness or disability or may have conditions requiring daily medical attention that prevents them from living independently. Our health care services include medical and non-medical care for individuals who need assistance with daily living activities such as dressing, bathing, grooming, toileting and eating.
Skilled Nursing & Rehabilitation

Skilled nursing is typically required for a short duration following surgery or complex medical needs requiring hospitalization. Our services can help an individual during recovery from stroke, surgery, fractures, wound care, and other illnesses. Willow Creek strives to help individuals regain their vitality and return home to resume their normal life, with a focus on an individual's health, activity function, and independence.
Social Services & Admissions

Social services are provided to assure that each resident can attain or maintain the highest practical physical, mental and psycho-social well-being. Our staff can help with admission procedures, potential plan of care, advance directives, and designating a health care proxy. Willow Creek dining services are open daily from 7:00 am to 7:00 pm.
