Dr. Goldfarb is an animal doctor who proudly served as an officer in the US army. After tours in Virginia and the Netherlands, Dr. Goldfarb returned to the US, and since 1974, he has been at Fairfax Animal Hospital. He purchased the practice in the mid-1980s and has watched proudly as it grew to become a long time staple of Northern Virginia's communities.

Our facilities are conveniently located in the Seminary Road Plaza, right-off Leesburg Pike (Rte 7) and Columbia Pike in Falls Church, also known as Bailey's Crossroads.

The required sections have a red * asterisk. You indicated that you were referred by a Current Customer and we would like to thank them! I assume responsibility for all charges incurred in the care of this animal. I also understand these charges will be paid at the time of service or release and that deposit may be required for surgical treatment.
Boarding Care Instructions

All dogs admitted to Fairfax Animal Hospital must be current on the following vaccines: Rabies, Distemper, Canine Influenza, and Bordetella. Describe all medications, dose, and frequency of use for all medications your pet is given, including over-the-counter medicines and flea/tick or heartworm medicines.
