Family First Mobile is made of Physicians, Nurse Practitioners that provide medical services to residents in senior retirement communities. Family First Mobile can provide a smooth transition of care from a skilled facility or hospital to care at home without a gap in care. A smooth transition requires personalized attention and intense coordination in order to ensure all the patient's needs for specialized care are identified and met.

Medical Records
Report
To request a copy of your medical records, you must sign a medical release form (see below). Once the form is received, records will be released within 30 days. You will receive a call when they are ready for pickup. There is no cost for records. A signed release must be faxed to our office from the requesting office.
Referrals
Report
Referrals can only be created when there is clinical justification. If you are needing a referral to a new office or specialty, you will be required to be seen at our office before the referral can be created. If you are requiring an updated referral to a specialty office, we do require a faxed request.
