Please bring an updated list of ALL medications you are taking OR bottles for these medications with you to each appointment. We understand that sometimes you cannot make arrangements for childcare, but especially for your Annual Physical Exam, we require that you make arrangements for childcare or have a responsible adult available to stay in the waiting room with your children.
Insurance

We ask that all patients present their insurance card at each visit. New patients must present insurance cards at their initial visit. If you do not have an insurance card or some proof of coverage, you will be responsible for full payment of the charges for that visit. You can then file your claim for reimbursement.

One of our doctors is always on call 24/7. If you need to speak to a doctor after hours, you may call our main number and follow the prompts. The answering service is responsible for paging the physician on call. We seek to accommodate the needs of our patients and will schedule your appointment at the most convenient time possible.
