A trained lifestyle coach works with you, showing how you can take charge of your health with tasty meals, reduced stress, and regular exercise. This event will be held online and you can attend virtually via your computer, tablet, or smartphone.

Our on-site Case Manager provides comprehensive care coordination to patients with the goal of improving an individual's health and social well-being. We are Community Partners with the PA Department of Public Welfare.
Our Case Manager utilizes the web-based program Compass, to determine eligibility and assist patients in applying for state programs such as Medicaid, Select Plan for Woman, SNAP, Cash Assistance and the Pennsylvania Low Income Home Energy Assistance Program (PALIHEAP).Case Management will aid the uninsured and underinsured in obtaining free or low-cost medication through a variety of web-based Patient Assistance Programs.
