Case Managers: Our case managers are all licensed social workers or registered nurses. The role of the case manager is to discuss the needs of the individual and determine how we can best serve those particular needs. Case managers are always available by phone and will periodically do scheduled home visits to re-asses your needs and determine how we can better help with the changing times.
