When you choose to book an appointment through our website, you will be asked to create a patient account. Next, you will be prompted to complete an Intake Form which will include information about who you are, why you would like our help, and your medical history. The process is quick and easy. We charge $25 for "no shows" or appointment cancellations with less than 12 hour notice, unless it's an emergency.
