The informed consent document must be printed in color. If you don't have color printer it can be filled out when you arrive for your appointment. We have a strict NO SHOW policy. The link in the confirmation email can be used to cancel your appointment online up to 12 hours in advance or we require 24 hour notice of cancellation by phone or email.
