Even though Swainsboro was our first site and houses our corporate office, we now serve 13 counties consisting of Emanuel, Bulloch Treutlen, Toombs, Tattnall, Jenkins, Candler, Appling, Wayne, Montgomery, Jenkins, Jefferson, & Evans County.

If you would like to participate in EGHC's sliding-scale fee program for people uninsured or under-insured, please fill-out the sliding scale fee packet below and bring your proof of income with you to your appointment. Don't forget to bring your insurance cards if you have insurance, a picture ID, and your proof of income (w-2, pay check stubs, letter from your employer, etc.) if you plan to participate in EGHC's sliding-scale fee program.

The patient-centered medical home is a way of organizing your care that emphasizes care coordination and communication to transform primary care into "what patients want it to be." Provide you with a list of community resources, such as food pantries, social services, support groups, etc.

Please keep in mind that our partnership with you will function ONLY if you provide us with a complete medical history and information about any care you have received outside of EGHC.If you are unable to keep a scheduled appointment, please call our office to reschedule so that someone else may take your place.
