Affordable Health
Formerly known as the Community Health Centers of Sarasota County, Inc., CenterPlace Health is a community-based non-profit Federally Qualified Health Center (FQHC). Since 2010, the non-profit has worked in partnership with the Florida Department of Health in Sarasota County (FL DOH-S) to provide primary care to low income and medically underserved communities.

CenterPlace Health is a Federal Tort Claims Act (FTCA) Deemed Facility, covered by professional liability insurance through our enrollment in the Federal Tort Claims Act Program.

The mission of CenterPlace Health is to provide efficient and effective delivery of quality, affordable health services to a diverse population in a compassionate and caring manner. CenterPlace Health is a Federally Qualified Health Center providing high-quality, low-cost health care to thousands of people at five locations in Sarasota County.
Preparing For Your Visit

As a new patient, you will want to provide the health center with a detailed and honest medical history of your entire family. It's good practice to arrive at least 15 minutes prior to your scheduled appointment. Be sure to keep all scheduled appointments with your provider. It's also important to give an update of any changes in your health and report hospitalizations during each visit.
Fees and Insurance

To see if you qualify for the sliding fee discount program, use the chart below to determine your income level based on your annual household income and family size. Individuals of a household both traditional and non-traditional are considered family. Families are tied together financially and CenterPlace uses Federal tax dependency to determine household/family.

CenterPlace Health is committed to serving all patients, regardless of their ability to pay. Although we do accept many healthcare insurance plans, patients who are not covered by insurance will not be turned away. Many patients are surprised to learn that they are eligible for low-cost healthcare programs.
Patient Bill of Rights
Report
Florida law requires that your health care provider or health care facility recognize your rights while you are receiving medical care and that you respect the health care provider's or health care facility's right to expect certain behavior on the part of patients. You may request a copy of the full text of this law from your health care provider or health care facility.
