Dr. Doherty launched CMOHS in the early-1980s to provide oral health services for disadvantaged children and adolescents who were unable to access dental care.

Before the Commonwealth Mobile Oral Health Services (CMOHS) team visits your location, we will need to distribute and collect dental consent forms. The dental consent form includes a medical and dental health history to be completed by the parent/guardian/patient (as appropriate). The CMOHS dental provider will review the medical history, take radiographs (as needed), examine the teeth and write a treatment plan for each patient.
