CODAC is one of Arizona's oldest and most respected community providers of specialty care for mental illness, addiction and trauma. What began as a grassroots drug abuse prevention program has grown into a multi-faceted organization that provides services across the entire spectrum of behavioral health care.

Patient Portal allows you to be an advocate for your own healthcare. You get easy and secure access to your health information as well as the ability to use online tools and resources to improve your health and wellness.
Billing and Insurance

During your first appointment - and as needed - you will meet with a Membership Specialist to review your behavioral health care benefits and any fees or financial obligations you may have at CODAC. CODAC participates with the following insurance insurance carriers for many products and services. However, not all doctors, nurse practitioners and therapists are contracted with each plan.
