If your work injury has had a permanent effect on one of various "rateable" body parts, you may be entitled to an impairment rating, also known as a schedule award. You are compensated for the loss of use of that body part. You must be alive to claim a schedule award; otherwise there is no time limitation.

Always keep a copy of what you submit, and make sure you have proof that you submitted the form. Complete the employee portion of the form and turn it in to your supervisor no later than 30 days following an injury. Make sure you get a "receipt of notice" from your supervisor. Be aware that workers are sometimes told to file recurrence claims when they should be filing new injury claims.
