Through the Donated Dental Services (DDS) program, we have donated over $30,000 in free case work; supporting the underprivileged, the elderly, and the medically fragile. Download our prescription form. To order hard copy prescription forms, peel and stick shipping labels, and additional shipping supplies, please visit our contact page. Separate individual cases in plastic bags, and staple the script to the outside of each bag for accurate processing. For our partners located outside Tampa Bay, you can easily request a FedEx shipping label be sent directly to your email.

Upload case photos for more accurate reproduction of your patient's existing dentition. Please remember to take clear and bright photos. For shade matching, taking multiple photos in different lighting situations is very helpful, as well as with shade tabs in place. You can upload up to six images per submission.
