This manual is designed to provide drivers, employees, and all other concerned parties with information regarding the operational policies, safety policies and general practices of SH Logistics, LLC. A wide scope of information is provided herein. However, it is not the intent of the company to list all of its programs, policies and or procedures in this manual.
Application
Report
Substance Abuse Testing Applicants extended offers for contractor or employee will be required to successfully pass a drug screen examination. In Connection with your employment or application for employment (including contract for services) with SH Logistics, LLC dba SH Transport, consumer reports may be requested from USIS commercial Services (USIS).
Driver Qualifications
Report
Must provide no less than 5 years of driving work history, non-driving work history for the past five years. No failed or refused drug or alcohol test in the last three years (this includes DWI's, DUI's and BAC's). Must have no more than four moving violations and / or accidents during the last three years.
Medical Qualifications
Report
Will take a thorough physical examination that meets or exceeds the FMCSA standards. This physical will include the requirements as spelled out by the FMCSA, These may include written screening tests as well as physical tests. If you are currently under any type of medical treatment, therapy, use of medical devices or medication, you are required by law to disclose this information to the doctor conducting the physical.
