New Patients

Please review your insurance policy requirements to determine if you need a referral from your primary care physician prior to visiting Sports Medicine Center. To expedite your check-in and keep waiting times to a minimum, we ask that new patients show up 15 minutes before their appointment. Your insurance card and driver's license - A copy of your insurance card will be requested at the time of check-in.

Our Sports Medicine Clinic offers an ambulatory or office based surgery facility. This surgery center was designed to provide safe and cost effective out-patient care for patients desiring surgical procedures. This will contribute to more effective patient care and greater satisfaction for the patient.
