Call the office and let them know you would like to make an appointment. You can download the client intake forms from this website and fill them out before the appointment or plan to come 30 minutes early and fill them out at the office. A first office call is about 2 hours in length and includes a complete history, possible physical exam and recommendations for treatment and a treatment plan.
