All biopsy specimens are submitted to the Oral Pathology Group at the Indiana University School of Dentistry. Performing a biopsy of your oral tissue is necessary to help diagnose a particular condition and provide you with the appropriate treatment. SPT or Periodontal Maintenance is started after completion of active periodontal therapy and continues at varying intervals for the life of the dentition.

A Referral Form from your dentist is appreciated, but not required. There is a Referral Form available for download which allows you or your dentist to obtain a Referral Form immediately. If your dentist does not have referral forms from my office, please let me know and forms can be mailed to them.
Dental Insurance

Please be aware whether the specialist you have been referred to is a provider for your insurance plan. Referral relationships may not be based on your insurance coverage. You may find out that you have a considerable out of pocket expense if the specialist is not a provider for your insurance. Please educate yourself about who the providers for your insurance are.
Post Op Instructions

Although not all of the following pertain to every type of surgery, you may find these instructions helpful. 1. CARE OF SURGICAL SITE: You should avoid eating and drinking for at least 2 hours after surgery. Smoking is not recommended, especially during the healing time. Do not rub your face in the area of the surgery.
