We need to acquire both medical and billing information to create an electronic chart and account for you. To make your appointment as efficient as possible and save you time in our office, you can download 2 separate forms: "Patient Intake Sheet" covers your billing information, "Patient History Form", covers your medical information, and "Patient History Form for 50+" covers your medical information if you are a patient over the age of 50.

New OB Class - this is a free 1 hour seminar held every other week on Wednesday evenings. The class is a thorough discussion for newly pregnant women and their partners about pregnancy management, nutrition and exercise in pregnancy, lab and genetic tests among many other topics. You will be offered the opportunity to register for this class at the conclusion of your initial phone call with our nurse.

We enjoy seeing young women ages 14 and older. Teens, although in general not requiring an involved physical exam or cancer screening, are well served becoming acquainted with the many resources a gynecologist can offer.
These health and lifestyle decisions might include, for example, a reminder to wear a bike helmet, to protect herself from STDs with condoms, how to evaluate herself and determine when to be seen for a vaginal infection or irregular period pattern.We want our teens to go out into the world with a complete knowledge of the gynecologist's role in her life.

Infertility is the inability to conceive after a year of unprotected intercourse, or the inability to carry pregnancies to a live birth. Fortunately, great strides are being made in understanding the causes of infertility in both men and women.

ParkWest Women's Health is pleased to offer AWAKE Liposuction of the abdomen and torso performed under local anesthesia utilizing the tumescent technique. Liposuction performed this way avoids the risk and expense of general anesthesia. In addition to their decades of surgical experience, Drs. William Harvey and Uma Penmetsa have special training in this low risk technique.
