Please arrive 15 minutes before your first appointment in order to complete the necessary paperwork and obtain your insurance information. You will need to bring identification, such as a driver's license or state ID card, as well as any applicable insurance card(s) and any co-pay and/or referral required by your insurance.
Medical Records

Both forms are required prior to the release of records. The fee for service for these items is likely not covered by your insurance and will not be submitted to the insurance carrier for payment. You are responsible for the payment of these fees. Payment is due at time of request. Once the full payment is received, please allow two to three weeks for completion of your request.
