Report
We at Performance Physical Therapy are now able to offer Telehealth Physical Therapy services to our patients. This involves one-on-one video conferencing with our physical therapists from the comfort of your home. With some exceptions, our therapists are able to offer most of the benefits of a regular physical therapy visit while you do not have to actually come in to the clinic at all.
What to Expect Your First Visit
Report
If it is your FIRST appointment, please arrive 10 minutes early to fill up the necessary paperwork, etc. It will expedite the process and will give you the most value for your visit with your therapist. Your first visit will be focused on the evaluation of your issue as well as some form of treatment related to your condition.
New Patient Packet
Report
Please take your time to complete the new patient packet and review our Privacy Practices and Financial Policy prior to your initial appointment. We still would like you to arrive 10 minutes early to your scheduled appointment time. Feel free to call us at (360) 714-0870 with any questions or concerns.
Insurance & Billing
Report
Thank you for choosing Performance Physical Therapy for your musculoskeletal and orthopedic care. We accept most medical insurances. If you don't see your insurance listed give us a call to see if we are contracted with that insurance company. It is always important to check with your insurance company to verify your physical therapy benefits and find out if you will have a copay and/or need authorization prior to your PT visit.
Direct Access/Self Referral Information
Report
Direct access allows the patient to be evaluated and treated by a Physical Therapist without being referred by a doctor or other healthcare practitioner for musculoskeletal issues. Sometimes direct access may be referred to as self-referral. YES! Physical therapists are trained to recognize "red flags" that may signal the need for more invasive medical intervention.
