His commitment to excellence in patient care extends far beyond southwest Florida, however. Dr. Daitch has been a strong advocate for fighting "pill mills" and in the development and implementation of E-FORCSE, Florida Department of Health's Prescription Drug Monitoring Program.

How to Begin

Once you call to make an appointment (239-437-8000), you will be emailed or mailed your New Patient Packet. Ideally, the paperwork will be received to APMSS prior to your scheduled first appointment. You should bring the completed New Patient Packet to your first office visit. Health insurance card (as well as the insurance card for secondary insurance, if applicable).
