Healing Hearts Pediatrics, P.L.C., follows the outlined Well Child Visit Schedule below. Please print, complete, and bring all age-appropriate forms to your scheduled appointment. Visits in parenthesis do not require paperwork to be printed off in advance. Authorization to Treat (requests for authorizing treatment other than that of the legal guardian need to be made in person and requested by the legal guardian).

Q: Will there be a copay for every visit? A: Depending on your insurance, a copayment may be required. You may contact your insurance company to check if your plan requires that we collect a copayment. Q: What if I am going to be late for my appointment? A: We request that you plan to arrive 10 minutes before your appointment, so that your child is in the room and ready for the provider for the scheduled appointment time.
