I am in private practice and operate as an independent practitioner located at 2364 N. Hwy 287 Suite 101 in Mansfield Texas in the Transitions Counseling and Case Management offices.

Insurance companies will require that I submit a diagnosis, progress reports and treatment plans that reveal your personal information, to get approval for future visits, and there is no absolute guarantee that information will remain private and confidential.
