Hometown Health Center (HHC) was established as Sebasticook Family Doctors (SFD) in 2003 when Sebasticook Valley Hospital was awarded a Federally Qualified Health Center grant. Prior to becoming an FQHC, the organization was a medical facility under Sebasticook Valley Hospital called The Family Doctors with locations in Dexter and Canaan.

In 2016, Sebasticook Family Doctors was renamed Hometown Health Center. HHC now operates three locations, Dexter, Newport, and a School-Based Health Center for RSU19 students at Nokomis Regional High. Our services have expanded and our goal of providing a high-quality and comfortable medical home for our communities has been met.

Patients are able to be seen by another provider within HHC if it best meets their needs or preferences. If needed, HHC will accommodate patients for urgent or routine care at another location within the organization based upon available extended hours. Our facilities and providers are accessible through extended office hours, minimized wait times, same-day appointments, weekend hours, on-call providers, and a patient portal.

Dr. Israel Adeloye, DMD, says he loves public health because he was once a recipient of public health services. He loves science and working with his hands and dentistry is a unique way of giving back. He went to the University of Illinois, Tufts School of Dental Medicine for Dentistry, and Tufts School of Medicine for Public Health.
