It is Choicecare Home Health's vision to be the provider of "CHOICE" by offering patients and families compassionate, caring and highly skilled home health services in the comfort of their home. We prefer to assign the same RN/therapist to the same patient, establishing a continuity of care and more importantly, gaining the patient's confidence and allowing for a personalized relationship.
Our focus is on meeting the unique needs of the patient and family. Our complete effort is to deliver quality care in the most cost-effective manner. We are on call twenty-four hours a day, seven days a week. Because we realize the patient apprehension often involved, we will meet with the patient and family members prior to hospital or nursing home discharge explaining their physician's orders.
