Home Health & Hospice Care Services Tahlequah, OK
Cherokee Nation Home Health Services is a tribally incorporated non-profit agency dedicated to delivering compassionate, high-quality home care services. Established in 1981 by the Cherokee Nation, we specialize in Medicare and Medicaid-certified home health, hospice, and outreach programs for elderly and disabled individuals. Our transition to an independent entity in 1997 under the Balanced Budget Act allowed us to maintain not-for-profit goals while adapting to evolving healthcare standards. We serve northeastern Oklahoma with a focus on patient dignity, offering skilled nursing, therapy, and support services in the comfort of patients' homes. Our hospice division provides 24/7 on-call nursing and holistic care addressing physical, emotional, and spiritual needs.Through state-licensed programs like the Oklahoma State Personal Care Plan and Advantage Waiver Program, we extend care to Medicaid-eligible individuals across 17 counties. Our team of registered nurses, home health aides, social workers, and volunteers ensures compliance with Medicare requirements while prioritizing patient comfort and family involvement. We maintain a near-perfect compliance record since obtaining hospice licensure in 2000. As the only locally owned non-profit hospice in Tahlequah, we combine cultural sensitivity with clinical excellence. Our service model integrates seamlessly with long-term care facilities while emphasizing home-based care whenever possible.

Services
Home Health Care
Our home health care program provides skilled nursing and therapy services for patients recovering from illness or injury. Certified professionals deliver intermittent care under physician supervision, including wound care, medication management, and mobility training. Services follow Medicare guidelines requiring homebound status and physician-developed care plans.
Hospice Services
Hospice care focuses on comfort rather than cure for terminally ill patients. Our interdisciplinary team provides pain management, emotional support, and spiritual counseling while respecting patients' religious beliefs. Services include 24/7 on-call nursing and bereavement support for families.
Outreach Programs
Outreach initiatives serve Medicaid-eligible individuals through Oklahoma's State Personal Care Plan and Advantage Waiver Program. Services include personal care assistance, home safety assessments, and coordination with Department of Human Services for eligibility determination.
Skilled Nursing
Registered and licensed practical nurses deliver specialized care including medication administration, wound care, and symptom management. Nurses develop care plans in collaboration with physicians and provide patient education for self-care management between visits.
Therapy Services
Physical, occupational, and speech therapy services help patients regain independence. Therapists develop customized exercise programs, teach adaptive techniques for daily activities, and provide equipment training to improve mobility and communication abilities.

This Place does not Need to be in business anymore! The PCA program Is a joke to them, As they Cannot Be responsible for showing up. And when you call they act like it's not their responsibility? They make us wait all day, only to not even a call. And when they actually do get someone to show up, do not get them to grocery shop, unless you enjoy taking over half the order back. What a waste of time! A BAD company to put up with

I truly enjoyed the AIDS and the nurses that were part of my Healthcare team the only problem I've had is with Administration and the office PPL. I feel that I was not valued nor would be heard when I had issues. I feel they need to truly reorganized the administration and their office personnel to treat the patients with compassion and respect.
