This should include notes from any office visits with your physician, lab results, x-ray results and operative reports from any gynecology related surgery. A written authorization to release your records is usually required. If you have had prior surgery, you should contact the medical records department of the hospital where the surgery was performed and request a copy of the operative report be sent to our office.
Pricing and Insurance

We recognize that infertility treatment is often not covered by insurance. Therefore we will strive to perform only those diagnostic tests and procedures that are essential and avoid extraneous testing that does not provide useful information that will alter our treatment plans. For selected patients without insurance coverage, we offer outpatient tubal reversal for a fixed price of $6800.
