Report
He received his bachelor of science degree from Hillsdale College in 1985. He completed medical school, a general internship and orthopedic surgery residency at the Chicago College of Osteopathic Medicine in 1995, which is now named Midwestern University. He has served multiple times as an oral board examiner.
Medical Staff

Christopher Dilger graduated from the University of Evansville with a BS in Biology in May of 1999. He graduated from the Butler University PA program with a BS in Health Sciences in May of 2003. He has experience working as an EMT, as well as in orthopedic/neurological environments since his graduation and has been working with Dr. Eby for six years.
Physical Therapy
Report
Beth graduated from Indiana University with a Bachelor of Science degree in physical therapy. She also has been educated in myofascial release techniques and McKenzie cervical and back rehabilitation. She has worked in a variety of settings including acute care, long term care, orthopedics and home health.
Insurance
Report
Primary Insurance: We will file claims with the patient's insurance upon the patient's submission of proof of insurance (i.e. insurance card indicating coverage, identification number and group number). In the event the patient has insurance coverage but cannot provide documentation, payment is due at the time of service unless prior arrangements have been made with the Billing Office.
