The majority of the providers seeing patients within our clinic are private practices and thus are separate entities from Sage. Effective Sept 1, 2021: We please ask that all patients wear a face covering while in our office.
We also require all our employees and providers to wear a face mask when they are not at their desk or can not maintain a social distance of 6 feet. You will see a section that says form and a link to Release of Information.
