Before we can dispense insurance billed items to patients, Insurance Companies require the following documentation. Please notify us if the item is an URGENT request for either patient discharge or home so we can dispense the item as soon as possible. 3 Wheeled Small Black Scooter can be billed through Medicare if All Mobility Requirements are met.
