We ask that patients arrive 15-30 minutes prior to their initial appointment time to complete registration paperwork. After this paperwork is finished and at the appropriate time, you will begin your appointment with your treatment provider. You can expect to have a thorough history taken relative to the reason for the appointment.
What is your payment policy
Report
It is expected that patients will pay their share of the treatment cost at the time of service. This includes the balance in full if the insurance deductible has not been met. We do accept self pay patients. If you are a self paying patient, it is expected that you will pay in full at the time of each session.
