Hometown Family Health is a Direct Primary Care (DPC) practice in Fort Collins, CO focused on two things, relationships and time. With DPC, we have taken out third party payers and any other barrier between you and your care. This allows us to take the necessary time to provide personalized care without the frustrations of a typical practice.

Acute manipulative treatments included in monthly membership (ie acute illness or injury). Chronic or frequent treatments available at an additional charge (Members: $150 initial evaluation and treatment/$100 per follow up treatment; Non-members: $200 initial evaluation and treatment/$125 per follow up treatment).
