With Summer approaching and kids being out of school, my schedule will look a little different so I can enjoy this precious time with my kiddos. They are growing so fast! A copy of the front and back of your insurance card would also be appreciated. A 24-hour notice is required for cancellation of an appointment, or you will be charged in full for the appointment. Payment is due before your next appointment. Exception: If you are feeling under the weather, have a cold, the flu or any contagious conditions, please contact me as soon as possible and I will reschedule you without any extra charge.

List all surgeries, injuries, & accidents with year of occurrence (including known triggers for your pain or discomfort.) I, the client, understand that the massage I receive is provided for basic purposes of relaxation and relief of muscular tension. I take full responsibility for informing my therapist of any condition that could effect this session.

In the event of a car accident, if your doctor recommends massage therapy as part of your recovery program, I am here to help you. I can assist you with billing your auto insurance company directly so you don't have to deal with the paperwork. I also have good referrals for physical therapy and chiropractic if needed.
