We will also make a copy of your insurance card and verify your chiropractic benefits for you. Once the paperwork has been completed, Dr. Chad will spend some time getting to know you and the reason for your visit.

We are located just off of highway 212 and Shady Oak Road right next to Wooddale Church, and within the same building as MidWestOne Bank. Enter the building through the main doors, and the elevators/stairs are right there. Go to the 3rd floor and our office is on the left just past the bathrooms.
