Click the Rx lab slip image below to print a lab slip. A completed lab slip with doctor's orders, signature and license number is required before we begin work. Wrap stone models to ensure they don't get damaged in transit. If sending more than one case, please bundle by stacking same-sized boxes and then tape together securely.
