Rates & Insurance

I am an in-network provider for several different insurance and medicaid companies. If I am not in-network for your insurance, you can pay the private rate, and I will give you the forms you need to file for reimbursement at the out-of-network rate. Medicaid does not reimburse for out-of-network providers.
