The Mission/Vision of the Family Health Center of Southern Oklahoma is to offer affordable, high quality care to our patients through comprehensive services. Community Health Centers are providers of primary health care to medically under-served populations. Community Health Centers focus not only on improving the health of individual patients, but on improving the health status of the entire community.

You will be asked to sign an authorization form for release of medical information and for your insurance company. All information you provide us is strictly confidential. If you are more than 15 minutes late for your appointment we will need to schedule you another day. Proof of income if you will be applying for the sliding fee (paycheck stubs or tax returns).
