Please note: Concord Pediatrics has a physician on-call 24 hrs a day to answer your urgent medical questions.
New Patients

Thank you for choosing Concord Pediatrics, P.A. as your child's medical home! We always welcome new patients. Our office staff will gladly inform you of the types of insurance that we are currently accepting. Please contact our office for additional information or to request a first appointment. Parents are responsible for obtaining the medical records from their child's previous provider prior to the first visit.

NICHQ Vanderbilt Assessment Forms are one tool we use to diagnose Attention Deficit Hyperactivity Disorder. If you have concerns about your child and ADHD, please discuss those concerns with one of our medical providers prior to using these forms. Symptom Checklist is designed to be filled out by an older teen or adult on their own.
